Expression of Interest
The LLG OHT is formulating a Population Health and Quality Improvement table and is seeking participants. The goal for this table is to embed a "Population Health Management" approach and quality improvement culture within the OHT. Through doing so we hope to bring more data driven conversations to the amazing work done within the OHT. Ideal participants for this group would include (not exclusive to):
- Leads, specialists, managers or directors of population health, public health
- QI, data, and analytics (Data Management Coordinators, Decision Support Specialist, Quality Improvement Specialist, QIDSS etc.)
- Those with an interest in population health data management, and or Quality Improvement
- As a group, work in a variety of sectors e.g., acute and rehab hospitals, primary care, community organizations, public health, congregate care settings, mental health
Time Commitment: The intent would be to meet either monthly or bi-monthly for 1-1.5 hours. The first meeting will be introductory and will give participants a chance to introduce themselves, review the intent/purpose of the meeting, begin crafting the vision, missions, and values for the table.
